NEW COLORADO PALLIATIVE CARE GUIDES
The guides are tools for consumer education on palliative care in Colorado.
It is consumer focused and specifically for people living with serious illness, and has three sections:
1) You are diagnosed with a serious illness – now what?
2) Living with serious illness.
3) Changing goals of care.
The Colorado Guides are based on the Washington State Palliative Care Roadmap, with additional contributions from Colorado Palliative Care Experts and patients. Please contact authors Jenn Klus, RN MPH, or Cordt Kassner, PhD, with questions or comments.
